The Internal Forces Of Brain Injury

External Force being Applied Against the Head, can result in Forces within the Brain that Can Result in Injury.

Accident Scene

Impact. Internal impact occurs when the brain hits the inside of the skull. The impact damage we are referring to is not the external impact of the head coming into contact with something, but the brain coming into contact with the skull. This can occur without a blow to the head.

Contusion

Shear. The brain is made up of many different layers of matter, each which have a different density (weight). The axons are long, thin brain fibers that cross these layers. When the brain is subjected to acceleration/deceleration forces (such as in whiplash), especially when such forces are centrifugal in nature, the different layers are accelerated/decelerated at different speeds. When axons are torn or stretched as a result of the different layers moving at different speeds, this is called shearing. Shear injury is a possibility in both the situations where the head hits something and when there is sufficient whiplash force against the brain. It happens much less likely from a blow to the head. It is the most likely mechanism of injury in an automobile wrecks, because of the rapid deceleration of the vehicle in the crash.

Understanding Subtle Brain Injury

The concussions that disable, are almost always more symptomatic at 24 hours, than at the 2-4 hour time frame when injured persons are evaluated in the emergency room. Brain injury symptoms escalate over the first 24 hours, because brain injury involves a cascade of events. It is critical that if you are still symptomatic the day after your injury, go back to the same Emergency Room, don’t wait for a doctors appointment. It is critical that the Emergency Room personnel see that the symptoms still persist or have gotten worse.
